Complaints should be lodged in writing using the Office of the Police Ombudsman Complaint Form. You can obtain a form in any of the following ways:

- Contact the Office of the Police Ombudsman and arrange for a complaint form to be posted or faxed to you

If you need help with writing a complaint or you wish to discuss your complaint the Office of the Police Ombudsman staff are available to help. The Office of the Police Ombudsman can also arrange interpreter services as needed.
There is no cost involved with lodging a complaint with the Office of the Police Ombudsman.
